The last High Sierra hotfix fixed everything that I am currently aware of, so as long as you go to the most recent release you SHOULD be OK. It’s a very big change under the hood though and a lot of file handling changed in XTension to support it so I can’t tell you that you won’t discover any other problems. Those that have updated to it and high sierra aren’t reporting anything new yet and I haven’t noticed anything here, but that doesn’t mean there isn’t anything else.

As far as updating, everything XTension needs is in it’s database, as Phillipe already said. 

Unless you’re running XTension naked in the Applications folder you should not have to worry at all about the other 2 folders he linked to. There is nothing important in the Application Support/XTension Support folder unless you’re running from the Applications folder in which case that is where your database will be. And the other one is not important at all and will just be re-created as needed by XTension.
